Membership Contract
Personal Care Physician is a month-to-month service which means you may discontinue your membership at any time by providing, in writing, notice of your cancellation. However, if you cancel your care but decide to re-join at a later date, you must pay a $99 re-enrollement fee. Other terms for re-registering, if applicable, will be discussed with you at the time you cancel your service.
Payment Options
Monthly care fees may be paid for by cash, credit card, or debit card. Quarterly, semi-annual and annual payment options are also available.
Registration
To become a Personal Care Physician patient, you pay a one-time, $99 registration fee. After you are registered, you pay an affordable monthly care fee based upon your age. For families, the registration fee is charged only once per billing account (not per person).
To register, simply complete our registration form located under PCP patient form and call or email to schedule an appointment. After scheduling your intial appointment you will need to have your registration forms completed. Our Wellness Staff will establish your patient and billing account, set-up your electronic medical records, coordinate the transfer of your medical records (if requested) and confirm your new membership. It’s that simple!
